Too often we hear the call for "teamwork," but then find the atmosphere is missing to cultivate and encourage people to work together.  Part of the problem arises from the fact, as pointed out by Harvey Mackay, best-selling author and chairman of the Mackay Envelope Company), that most employees do not really know their coworkers.  Mackay has put together two forms to help managers and employees better understand each other so that they can more easily mold themselves into a cohesive team.
